Transition
For young people with learning disabilities, complex needs and autistic spectrum conditions, the transition from childhood to adulthood can be an extremely difficult time. Service provision often changes and this can lead to some young people being affected by gaps in support.
Consensus’ provides a high quality service that ensures that young people and their families receive consistent support of the highest quality throughout the transition stage.
Supporting individuals with a variety of needs including people with autism, physical and complex health needs and moderate to severe learning disabilities, Consensus is focused on partnership working to achieve the best. Rather than young people coming to a brand new service and having to start again, Consensus staff spend time at their existing services, or in schools, learning about their unique stories and histories.
Our staff ensure that none of this vital information, about someone’s personal successes and achievements, as well as the challenges they face, is lost. Working closely with families, carers, teachers and support workers, transition staff devise personalised support that is right for their needs.
We also provide intensive support for people who have been affected by changes to the support they are entitled to, or gaps in service provision.
One person who has been supported through a transition is David. To read his story please click here.
