Supporting People
The involvement of the people accessing our services is an integral part of the preparation of a Consensus Support Plan. People are encouraged to work closely with the support team in setting their objectives and the nature of services that are required to achieve these objectives.
Consensus has developed a range of communication formats to ensure that support plans are accessible and can be easily understood by everyone. This includes the use of symbols, audio/visual and multimedia interactive technologies.
Support Plans have been developed to incorporate “O’Brien’s” Five Accomplishments:
Community Presence
To be engaged within the local community developing positive relationships with people who do not have a learning disability
Choice
To be supported and encouraged to make informed choices about lifestyle
Competency
To be supported in participating in new activities and maintaining their existing hobbies
Respect
To have the right to be valued
Community Participation
To be supported in accessing the local facilities and amenities within the community
A Consensus Support Plan embraces the ethos of Valuing People, the British Institute Of Learning Disabilities’ (BILD) ten outcomes and always takes a person centred approach.
1. I make everyday choices.
2. I make important decisions about my life.
3. People treat me with respect.
4. I take part in everyday activities.
5. I have friendships and relationships.
6. I am a part of my local community.
7. I get the chance to work.
8. People listen to my family’s views or the views of other people who are important to me.
9. I am safe from bullying or abuse.
10. I get help to stay healthy.
