Assessments and Admissions
Consensus has developed a comprehensive assessment programme to ensure that we can determine a person’s needs and compatibility prior to an admission to one of our services.
This includes:
A Direct Interview with the person accessing a service, their family/advocates, social and healthcare professionals, care managers and commissioners
A comprehensive review of each individual’s health, clinical and medication reports from previous services including schools, colleges and care services
An initial support plan and breakdown of costs
A pre-admission support programme
Throughout this process the Service Manager, intended Key Worker and Referral Manager work in partnership with the person using the service and other parties to ensure a smooth transition to the service of their choice.
Consensus also has the appropriate resources and Infrastructure to support Emergency Admissions and Placement Breakdowns.
